Involve Haitians in Closing the Gap is a comprehensive neighborhood based education program focusing on cardiovascular diseases and diabetes. Through home visits, in office counseling and support groups, we provide screening, health education, and client follow-up services for Haitians in Palm Beach County.

Funded by the State of Florida Department of Health and the Community Foundation of Palm Beach and Martin Counties, Involve Haitians in Closing the Gap has received “Best Practice” recognition every year since it began in 2001.
